Wellness Clinic Program


The Wellness Clinic offers education and preventative care opportunities for the better health of our community. Community partners, health professionals, and volunteers collaborate to provide the highest quality programs and services that meet the health and wellness needs of the residents of our area.

 

To Be Eligible for Services:

  • Eligibility will vary depending on the program or service

 

Services Provided:

  • Well woman visits

  • Health fairs

  • Screening and testing

  • Vaccinations

  • Educational programs

  • Healthy lifestyle programs

 

Cost of Services:

Many Wellness Clinic services are free to the public or are at a low cost.

Health Clinic Program


The Health Clinic offers services in an outpatient setting under the supervision of a Medical Director and Certified Registered Nurse Practitioners. Patients readily have access to primary and preventative health care, diagnostic lab work, and assistance with needed medications and other resources.

 

To Be Eligible for Services:

  • Persons must be 18 – 64 years of age

  • Employed, actively seeking employment (as verified from the employment office), or enrolled in school

  • Without a regular healthcare provider and without healthcare benefits

 

Services Provided:

  • General physical examinations

  • Treatment of common illnesses and chronic diseases

  • Women’s health (including gynecologic exams, clinical breast exams and referrals for mammography services)

  • Patient education

  • Preventive care

  • Laboratory diagnostics

  • Medication Assistance

  • Diabetes Assistance

  • Patient referrals for necessary care that exceeds our scope of practice

  • Physicals for employment or school

 

Cost of Services & Operations:

  • Health Visit:  $20

  • Diagnostic Lab-work:  $15 per lab

  • Medication Assistance Program:  administrative fee $5 per month per prescription

  • Diabetes Test Strips: $10 per box of 50

 

Hours of Operation:

Health Clinics are delivered Monday, Wednesday, Thursday & Friday 9am-1pm. Call for an Appointment.  (NOTE:  Hours subject to change due to COVID or other variables. Please, call to verify availability for services.)

 

Appointments can be made by calling the clinic at 256-760-4713. The clinic will usually also accept walk-ins.

Dental Clinic Program


The main objective for the Dental Clinic is to alleviate pain and provide self-sufficiency to our patients through tooth extractions. Services for the Dental Clinic are available to anyone in need regardless of employment status.

 

To Be Eligible for Services:

  • Persons must be at least 18 years of age

  • Without a regular dental care provider and without dental care benefits

  • If taking a blood thinner medication, must have your doctor’s permission and have not taken the medication in 10 days prior to the extraction

 

Services Provided:

  • Consultation and Exam

  • Panoramic x-rays

  • Tooth extractions

  • Medications

  • Referrals to dental specialists

 

Cost of Services:

  • Dental Clinic: Cash only – $60 for the first tooth extracted; $20 for each additional extraction

  • Vouchers valued at $40 to be applied to the first extraction can be obtained through the Help Center (621 S. Court St. Florence) for Lauderdale County residents and at the Colbert Caring Center (108 N. Water St. Tuscumbia) and the Sheffield First Presbyterian Church (130 E. 5th St. Sheffield) for Colbert County residents.

 

Hours of Operation:

The Dental Clinic strives to provide services on Fridays when there is a Volunteer Dentist available. As the Dental Clinic is operated under the care of Volunteer Dentists, it is recommended you call to confirm the calendar for upcoming dental sessions. 

 

The Dental Clinic does not take appointments. Patients are seen on a first-come, first-serve basis on the days when dental clinics are held.  It is recommended that you arrive early to get a place in line. The clinic opens at 7:00 AM and the first 10 patients in line are admitted.
