Where are you located?

Directions:

The Shoals Community Clinic is located in the Handy Place public housing complex in West Florence at the corner of Perry St. and Lincoln St. GPS locater is 399 Perry St. The second entrance to the clinic parking lot is off 326 Wallace St. across from the St. James M.B. Church.

 

Coming from Sheffield / Colbert County (US Hwy 43):

  • Once you cross O’Neal Bridge into Florence, the street becomes Court Street. 

  • From Court Street, you will turn left onto Tennessee Street and proceed until the street ends at Locust. 

  • Turn right onto Locust. 

  • At the 4-way stop, turn left onto West Mobile Street. 

  • The third street down (on the right) is Perry, turn right. 

  • Go to the top of the hill and bear left into the clinic’s parking lot.

 

From Florence Blvd. (US Hwy 72):

  • Go west onto Tennessee Street and proceed until the street ends at Locust. 

  • Turn right onto Locust. 

  • At the 4-way stop, turn left onto West Mobile Street. 

  • The third street down is Perry, turn right. 

  • Go to the top of the hill and bear left into the clinic’s parking lot.

How do you qualify for services?

For the Health Clinic:

  • Persons must be 18 – 64 years of age

  • Employed, actively seeking employment (as verified from the employment office), stay at home mothers of pre-schoolers, or enrolled in school

  • Without a regular healthcare provider and without healthcare benefits

 

For the Dental Clinic:

  • Persons must be at least 18 years of age

  • Without a regular dental care provider and without dental care benefits

  • If taking a blood thinner medication, must have your doctor’s permission and have not taken the medication in 10 days prior to the extraction

How much does it cost to see a healthcare professional?

 

Health Visit: $20

Diagnostic Lab: $15

Medication Program: $5 per month per prescription

Diabetes Test Strips: $10 per box of 50 (meter is free) for patients only

Tooth Extraction: $60 first extraction (includes x-ray; medications; & consultation and exam); $20 per additional extraction

What are your hours?

 

The clinic offices are open Monday, Wednesday, Thursday & Friday 9am-1pm.

Call for an Appointment.

Health Clinics are Monday, Wednesday, Friday, and some Thursdays 9am-1pm or by Appointment

Dental Clinics are Fridays when Dentists are Available – No Appointments; First Come First Serve; Admission is at 7:00 AM

NOTE:  Hours subject to change due to special conditions with COVID or other variables.

What do I need to bring with me for my appointment?​

 

First time Health Clinic patients will need proof of employment or enrollment in school.

Generally patients will need to provide contact information, health history, and list of current medications.

Who is the doctor?

 

Health Clinic patients will be provided care by Certified Registered Nurse Practitioners. The CRNPs are in a Collaborative Agreement with Dr. Timothy Ashley who has his own practice away from the clinic. The Nurse Practitioners are certified by the Alabama Board of Nursing and can provide all primary health care needs.

Do you take checks, credit cards, or debit cards?

 

Dental care is on a cash basis. The Health Clinic will take cash or checks. We currently do not have the means to take debit or credit cards.

What dental services do you provide?

 

Tooth extractions only.

Who is the dentist?

 

Dentists who work in the Dental Clinics are local professional dentists with their own practices and/or professional standing. There are approximately a half dozen local dentists who volunteer at clinics.

Will the dentist pull more than one tooth?

 

The number of tooth extractions performed at the same visit is up to the judgement of the providing dentist. If a patient needs to return to a later Dental Clinic for additional extractions, the cost will still be $20 per extraction provided the x-ray is still current.
